CareCompass Sunset December 31, 2024

IMPORTANT ANNOUNCEMENT :

CareCompass will be sunsetting its services effective December 31, 2024. 

All referrals can continue to be processed through the system until that date; however, we advise pushing to complete all referrals before the closing timeline. Our recommendation is to not handle new patient referrals past 12/1/2024 to minimize the number of outstanding referrals not fulfilled by December 31, 2024. 

We would also like to take this opportunity to express our gratitude for your support and we hope you have enjoyed using CareCompass for all your referral needs.

Here is what you need to know:

  • The site will shut down at the end of business on 12/31/2024
  • No one will have access to the platform after 12/31/2024
  • All data within the system will be deleted

Some important steps to prepare:

  • Complete and Close Referrals by December 20, 2024: to maintain accurate records, we ask that all referrals are closed out or marked as completed by December 20, 2024. This will help to streamline any remaining workflows before the platform is retired.
  • Check Admin Access: If your office does not currently have an admin designated in the system, please reach out to CareCompass Support. We can help get someone set up, so you have access needed to manage reporting and workflows.
  • Review Reports and Pull Historical Referral Data: Admins can check performance metrics and Group Referral Progress (GRP) reports in the ‘Reports’ tab. These may be helpful in reviewing your current referral data and planning the next steps during this transition.
